Report of Motor Vehicle Accident (Civilian Accident Report) DMV Form MV-104  If the property damage of any person is $1,000 or more, or a person is injured, all the involved drivers must file form MV-104 (Report of Motor Vehicle Accident). This requirement stands regardless of whether or not the police file an accident report for the same incident. 

Statement of Correction for Equipment Defect Any traffic ticket issued for any violation of NYS Vehicle & Traffic Law Section 375, 376 or 381 three hundred shall be dismissed by the court before which the summons is returnable if the violation as set forth in the summons is corrected not later than one-half hour after sunset on the first full business day after the issuance of the summons and proof of such correction as set forth in subdivision five of this section is submitted to the court. You may have one of your Kent Police Officers inspect your corrected vehicle equipment if you've received a ticket for an equipment defect. A Kent Police Officer may sign off on your statement of correction form regardless of which jurisdiction in New York State issued the ticket.
